A 63-year-old man presented with left flank pain, night sweats, and weight loss of 7 kg.

  • CT: 11.5-cm left renal mass. No hepatic or pulmonary metastases
  • Labs: Hgb 11.0 g/dL, calcium, LDH, and creatinine are normal
  • Surgery: Left radical nephrectomy
  • Pathology: Fuhrman grade 3 clear cell carcinoma with moderate tumor necrosis invading the left renal vein. Metastatic carcinoma in 1 perinephric lymph node (pT3N1M0).
